Mail Donor Form

The Sean Hanna Foundation is a public charity registered in New Jersey, exempt from federal income tax under section 501(c)(3) of the Internal Revenue Code. Contributions are tax deductible. Please use your paypal receipt as proof of your donation. Upon request, we will provide a receipt for contributions sent through the mail.

For Corporate Matching:
Many companies have programs through which they match the charitable contributions made by their employees. Through corporate gift matching, your employer can multiply your gift to SHF, giving it even more of an impact in changing the lives of children with cancer.

Please check with your company's Human Resources office to see if they offer matching of charitable donations. (If they don't, you may want to ask your company to start one.) If your employer does offer this program, your Human Resources department should provide you with a matching gift form to send to us along with your donation. Complete this form and mail it with your donation to the address listed above.

We will verify receipt of your gift and return the form to your company for matching funds.
